How the Program Works

Each family is assigned a Community Support Case Manager who provides intensive mental health services and support. With the family, the PEP Connections case manager brings together people invested in a child’s life to create and implement a plan of care that will support the child and the family.  These highly experienced professionals take the lead in coordinating a plan that builds on the child’s and the family’s strengths.  The plan is developed by an inter-system treatment team that includes the child, the family, family support persons (e.g. an involved uncle, a regular babysitter, etc.) and any professionals who are actively working with the family, (e.g. DCFS Case Worker, psychiatrist, school professionals, etc). 

This plan builds on what is working in a child’s life and emphasizes the family’s strengths.  It identifies the child's and family's needs, as well as the supports and services necessary to meet those needs. This coordinated approach ensures that everyone invested in the child’s life is on the same page thereby eliminating confusion and encouraging success.  

PEP Connections case managers think creatively and work collaboratively with the family and other professionals.  The goal is always the same - to put needed supports in place to assist the child to live successfully in the community and reach the highest quality of life possible.

 

An important strategy in liaison work is to teach the liaison function to one or more adults who are important in the life of a child, and to the adolescent when old enough and competent, so that the ecosystem can function effectively and meet new crises without external assistance.
